If you have not already done so, please submit your Daily Health Screening Form immediately. This is a reminder that health forms must be submitted EVERY DAY (even on days that the students are at home) for all hybrid K-8 and all PreK students, no matter whether they attend 3 or 5 days a week.

Forms should be submitted by 8:00 a.m. for hybrid K-8 students and completed no more than an hour before the school day begins. Forms for afternoon PreK students should be submitted by 11:00 a.m. and completed no more than an hour before the school day begins.

There are still forms outstanding for today. It is critical that we track the health of our student population. We thank you for your consideration and cooperation in this matter.
