Important Health Forms for Students Transferring to Madison Junior School

Dear Parents/Guardians:
In the attachments below, please find an Important Notice regarding Pre-Participation Athletic Examinations for participation in the MJSAO Dodgers Athletic Program, as well as, the Complete Athletic Packet mandated by the NJ Departement of Education. 
The Complete Athletic to be completed by the student's physician, athlete, and parent.  
Physical Examinations must be conducted by your child's physician within 365 days of the initial start date of practice.  Fall sports are scheduled to begin on September 6, 2018.  In order for your child's physical to be valid it must have been completed no earlier than September 7, 2017.  
All completed paperwork should be submitted to the school nurse by the stated deadline or to the main office during summer vacation.  Your student will be wait-listed on the MJSAO website by the MJS Athletic Director once you register until the school physician reviews all forms as mandated by the NJ Department of Education.  Forms are reviewed periodically in the summer, so the earlier your forms are recieved the quicker your child will come off the wait list.  This process can take a few weeks once your r forms are submitted until they are approved by the school physician.  
For those students with valid physicals that are still in date (no earlier than September 7, 2017 for fall sports) and on file at the nurse's office an Updated Health History  must be completed along with the review of the Opioid Fact Sheet and the Opioid Signature Sign Off Sheet below.
