Important Information About Vaccinations for 6th Graders

To the Parents of all Sixth Grade Students,

I hope you are all doing well and that you are having a great summer!

This is just a quick reminder that all students entering the 6th grade are required to have their vaccinations up-to-date for entrance into school.  The vaccine updates that are required prior to entering the 6th grade are the Tdap and the Meningococcal vaccines.  Please check with your child’s pediatrician to make sure your child is up-to-date on all of their vaccinations as soon as you can.  In addition, please provide a copy of their vaccination record along with an Annual Pre-Participation Form from your child’s pediatrician’s office prior to the start of school. 

The Annual Pre-Participation Form can be found on the school website under the Parent tab.  In the drop-down menu choose “Nurse” and look for the appropriate forms.  Please read the letter that is in the Nurse drop-down menu prior to going to your child’s pediatrician’s office to be sure you have all the forms you will need.  The medical forms and vaccination record can be left in the mailbox outside of school by the main entrance doors.  Once that external mailbox is emptied, Bridget or Mary will place them in my mailbox.  Please know that visitors are restricted at this time, so you will not be able to enter the school.  You can also scan and email them to me if that is better for you. 

I understand that some pediatricians are still seeing patients on a limited basis, but if you can at least schedule your child for a visit for the administration of the Meningococcal and Tdap vaccines, that would be best.  That is just a nurse visit at most places.  If you have any questions feel free to reach out to me at my contact information below.  At this time email works best for me rather than a voicemail message, as I am not often in the office at this point in time due to the restrictions on visitors at school.
