Player/Parents

The Player Code of Conduct should be
printed, read and signed by both the
players and their parents or guardians.
Please note that any parent or guardian
that wishes to watch a game must sign
the form.
Signed forms are to be returned to the
sports box no later than 1/13/06.
Coaches

The Coaches Code of Conduct is to be
printed out, read and signed by every
coaching staff member.
The completed forms can be brought to
SVMS and dropped off in the Sports Box
or left at the front desk for Kelly in
5A to bring home to Tom Miller.
